Care Provider – Direct Care Provider – Direct Support Professional
Job Title: Care Provider – Direct Care Provider – Direct Service Professional
Compensation: $21/hour
Bonus: $500 bonus after 90 days of documented Direct Care Services in good standing
Employment type: Part-Time or Full-Time
Expected hours: 15 – 40 per week
Schedule: Day shift

My Horizon is seeking an individual to provide direct care doing Day Hab, and/or Supported Employment, In-Home or Supported Living for individuals on the Home and Community Based Waiver in our local Fairbanks North Star Borough community.
The employee we seek must have a positive attitude, great work ethic, be honest, on time, reliable, and a team player. We believe that each person with intellectual and/or physical disabilities wants to be successful, just like anyone else. Our role is to provide enjoyable skill building experiences, creating a positive learning environment for living skills, social skills, exercise, nutrition, computer skills, arts and crafts, and self-care, in a safe and supportive way.
A safe and reliable automobile and proof of insurance is a condition of employment!
Employee must be on-time, maintain regular attendance, and have a positive attitude, no exceptions!
Applicants must:
- Be at least 18 years of age.
- Have a HS Diploma, GED, or be able to pass a test given by the Program Administrator.
- Be able to pass a fingerprint criminal background check, paid for by My Horizon.
- Have reliable transportation
Responsibilities:
- Provide direct care and support to individuals with disabilities.
- Ensure the well-being and safety of participants.
- Provide companionship and engage in meaningful activities with participants.
- Assist with transportation needs, including driving participants to appointments and/or community venues.
- Possess the ability to follow instructions and adhere to policies and procedures.
- Follow all HIPAA regulations to maintain the privacy and confidentiality of participant information.
- Have strong communication skills and the ability to work effectively as part of a team.
In addition:
- Experience is desired, but not required.
- Experience in using the Therap system is a plus, but not required.
- Pay depends on experience.
